EVIDENCE BASED PRACTICE
By:- firoz qureshiDept. psychiatric nursing
DEFINITION EBPEBP as using the best evidence available to guide clinical decision making .An integration of the best evidence available, nursing expertise , and the values and preference of the individuals families and communities who are serve
steps Steps for process of evidence
based practice Formulating the clear question
based on a clinical problem literature review to search best
available evidence analysis of strengths and weakness
of evidence
Cont.. use of based evidence in clinical
practice evaluating the efficacy of empirical
evidence
BARRIERS OF EBP Insufficient time for nurses to
implement new ideas. Lack of authority to change patient
care procedures. Lack of knowledge and confidence
among nurses and other professional practitioners.
Lack of continuing education process.
Cont
Lack of interest. Lack of time. Lake of support
NEW WAYS OF PRACTISING N
The advent of evidence-based reviews of nursing interventions makes visible for the first time in a systematic and critical format, the efficacy of one type of nursing intervention Over another. Systematic reviews that produce practice guidelines or inform clinical practice manuals have the capacity to direct nursing practice
Cont…or resolve issues where practices
differ according to scientifically validated evidence. Increasingly, the efficacy of a number of nursing interventions will become known and able to be ranked in a quantifiable manner. This identification, measurement and review of nursing work makes up the object domains of nursing that enable future government to take place.
From evidence to actionChanging any part of the health care
systemrequires careful thought and
planning; an individual nurse’s decision to change
anelement of practice can rapidly have consequences for colleagues and
patients. It can be challenging to turn
research findings into practise.
Cont..
There are many barriers to the effective translation of research findings into action including barriers which exist atorganisational levels, including economic, social and political
barriers.
Cont.. As the nursing profession expands
andnurses further develop their roles,
both independently and within
interdisciplinary teams, it is important that nurses
are able to challenge each other and hold each
other to account.
cont…The use of evidence allows nurses to
account for their decision making and justify
it accordingly, as well as allowing
practices to be reviewed and evaluated
objectively
EBP to initiate breast feeding:
An institutional changes in maternity care
practices effectively increased breastfeeding
initiation and duration rates. These changes
can be part of a comprehensive set of
changes, such as those implemented in
pursuit of BFHI (Baby Friendly Hospital Initiative) designation. They can be individual interventions such as
cont Increasing the rooming-in of
mothers and babies or discontinuing polices that are not evidence based.
Educating hospital staff through a 3-day training program has been shown to enhance compliance with optimal maternity care practices and increase rates of breastfeeding.
Cont.. Use of pacifiers by newborns is
associatedwith fewer feedings per day and a
shorter duration of breastfeeding overall
1. The main objectives evidence based
practice is; to increase awareness of the effectiveness of decision and action of practitioner s educator and policy makers. 2. In the health service it is designed to improve decision making and
achieveclinical effectiveness by enabling clinicians to use the most effectiveness
intervention,therefore reducing waste and eliminating
unnecessary practice.
OBJECTIVES OF EVIDENCE BASED PRACTICE
Cont..3. With the dissemination of the best
evidence to the widest audience imaginable with the use of information technology, it is expected that variations in the provision of services and patient s outcomes will be reduced.
4. The development and use of clinical guidelines based on evidence can also to achieve this objective.
Cont..Finally it also aims to reduce the reliance
on expert knowledge and increase t
ransparency of decision making. With the
evidence in the public domain lay persons
can, potentially, look for and use the
evidence to inform discussions with health
professionals.
USE OF EVIDENCE BASED PRACTICE
The idea behind the use of Evidence-based
practice or evidence-based healthcare is
that it comes from 3 combined aspects - the
values and preferences of the patient, the
clinical or clinical expertise, and the research evidence itself.
The combination of these three aspects is
what leads to improved individualized patient
outcomes, making evidence-based practice
crucial to nursing practice itself. it was
discovered that nurses use experience and
advice from their colleagues to guide their
practice, and this may not always be rooted in
evidence.
Cont..
However, despite having the research ready
for nurses to access, the nurse needs to have
a certain level of expertise in researching in
order to benefit from evidence and use it,
rather than rely solely on experience and
adviceAside from access, nurses must be
able to not only read the language used in the
research, but critique its usefulness, synthesize the findings and interpret
existing practice guidelines
Cont..Essentially, nurses are called to use
theirskills in finding research that is
relevant and useful to clinical situations in which
the patient may benefit, and incorporate
them into individualized patient care, as
well as establish health policies. It is not
enough to rely on nursing textbooks, using
what was taught in nursing school, or
practicing by observation or advice.
Role of nurse in evidence based practice1. Better patient outcomes; Nurse
regularly make decision on what care to provide for their patient .These decision can impact the patients health negatively or positively
2. making decision based on knowledge that is backed by research makes it easier for a nurse to doctor what care provide to her as opposed to trying something that may or may not be beneficial to patient
Cont..3. Decision making; using EBP to
provide care to patients increases the nurses confidence .This is because she knows that she is basing her decision about patient care on valid information that has been thoroughly researched.
Cont…4. Incorrect Advice; reliance on the
expert advice of colleagues can mean the application of out-dated or incorrect advice. individualised interventions evidence based practice encourages, tailored , and individualized treatment of client and emphasize care and out comes
